Registration
To register at Clay County Hospital, please have the following information:
- Your Medicare or Medicaid card
- Your Social Security Number
- Your employment information
- Emergency contact information
For faster, more efficient service, please bring your physician's orders
for outpatient testing.
You may register at Clay County Hospital:
Monday - Friday 7 a.m. to - 5 p.m.
Registrations Monday - Friday after 5 p.m. can be made in the ER department.
Emergency registration is located through the ER entrance and is available
24 hours a day.
