Patient's Bill of Rights 

As a patient you have the right to: 

  1. Considerate and respectful care.
  2. Information from your care givers about your diagnosis, treatment and prognosis in terms you understand.
  3. Information you feel is necessary to give informed consent prior to any procedure or treatment.
  4. Have your pain managed appropriately: (you can expect) information about pain and pain relief measures, concerned staff, committed prevention and management of your pain, state of the art pain management delivered promptly from health professionals that believe your reports of pain.
  5. Refuse treatment and be informed of consequences of refusal.
  6. Consideration of your privacy concerning your medical care.
  7. Expect all communications and records of your care will be treated confidentially.
  8. Expect that the hospital will make a reasonable response to your request for service.
  9. Obtain information about this hospital's relationships with other institutions as it relates to your care.
  10. Be advised of any human experimentation this hospital is engaged in if it affects your care or treatment.
  11. Expect reasonable continuity of care.
  12. Receive an explanation of your bill regardless of your payment source.
  13. Know what hospital rules and regulations apply to your conduct as a patient.
  14. Have spiritual care provided and to pursue your religious beliefs as long as it does not interfere with the rights of fellow patients.

Patients Responsibilities:

You are responsible to: 

          Ask Questions

  • The Doctor or Nurse may not know when you do not understand, or are uncertain, or need more information.

  • Ask your Doctor or Nurse what to expect regarding pain and pain management.

  • Discuss pain relief options with your Doctor or Nurse.

  • Work with your doctor and Nurse to develop a pain management plan.

  • Ask for pain relief when pain first begins.

  • Tell your Doctor or Nurse if pain persists and if you have any concerns about taking pain medications. 


Give Full Information

  • Tell your caregivers everything you can about your condition, including all symptoms, medication, and previous illnesses. 


Respect Others

  • The staff and other patients deserve your respect and courtesy. 


Plan Carefully

  • Schedule tests, optional treatments, etc. in advance. 


Follow Your Doctor’s Instructions

  • Take all medications as prescribed.

  • Listen carefully to instructions about your care such as diet and exercise.

  • Tell your Doctor if you are having trouble following instructions. 


Report Any Changes

  • Let your Doctor know immediately about any changes in symptoms, medications, or general condition.


Keep All Appointments

  • Be on time for all appointments.

  • If you cancel do so as soon as possible, give 24 hours notice at least. 


Pay Bills Promptly

  • Late payments increase overall costs.  Help keep costs down by always paying bills on time. 


Identification

*  Wear hospital provided name band until discharge.
